Improving outcomes by building better systems Health Quality Partners (HQP) is a not-for-profit health care quality research and development organization dedicated to improving population health outcomes through health system redesign and advanced care coordination. We design and test new ways to better integrate the disciplines of public health, quality improvement, care coordination, and systems thinking with care rendered by medical service providers. The aim is to create a health system in which the whole is greater than, rather than less than, the sum of its parts. In addition to better health outcomes, control over health care costs is supported two ways:
- More efficient and effective delivery of medical services
- Improving health and preventing disease reduces the rate of growth in health care services
System redesign work is done through collaboration with healthcare providers, members of the service populations, and other key stakeholders as appropriate including payers, employers, community leaders, health plans, government agencies, and local health departments. Aims, objectives, interventions, team models, and process specifications are established collaboratively. Target populations are defined. Process and outcome measures are specified. Data collection and analysis using sound statistical methods ensure that learning and improvement can be sustained over time. |
