Health Quality Partners is a not for profit 501c(3) organization committed to large-scale improvement of our health care system 

We currently provide direct nurse care coordination / care management services to over 900 vulnerable older adults with chronic illness - improving their health and maintaining their functional independence through 1:1 nurse contacts, group programs, and close collaboration with their physicians and hospitals.   

Along the way, we identify better ways to deliver such services, test these new approaches through rigorous research, and share our findings with others - through direct collaboration and through publications and conferences.

Current funding sources permit us to serve only a small fraction of those in our region who could benefit greatly from these services.  Our current funding level is also inadequate to ensure that our research can be completed and disseminated in ways that could help many, many thousands more individuals in the U.S. and around the world.
