HQP is developing a suite of tools and support services to assist organizations to implement HQP's community-based chronic care management program with high reliability and consistent effectiveness. The services, protocols, and processes, along with the tools, documentation, and educational learning modules we use to train our nurse care managers could be available soon--with help from you.  An online database for data collection and advanced statistical process control analysis and reporting, both specifically designed to support our program is also under active development.

We are actively looking for; 
 
1) replication partners who would like to co-develop these capabilities with us in order to deploy our model of community-based chronic care management 

2) funding sources to support this work; foundations, corporations, and individuals interested in helping us transform health care

HQP Learning Center

 
This secure website provides a set of training modules and online courses with test evaluations and trainer feedback to ensure that staff fully understand how to fill the care manager role.  It is currently under development by our team and could be available for use by the end of 2010, if support for development were provided.

HQP Care Management Database

We have successfully developed and deployed a 'home grown' database for the past 8 years designed specifically to support effective chronic care management for older adults.  This same application is now undergoing further improvements and migration to a scalable, secure, web platform.  Expected to be available for use mid 2010.

HQP Team Resource Center

This secure website provides a large set of tools and materials for care managers and program managers.  It is currently under development by our team, but should be available for use by our replication partners mid 2010.

HQP Advanced Analytics Available NOW!

We have extensive experience developing process performance metrics and implementing them using statistical process control analysis.  Dashboards show a care manager patients that need additional follow-up, when to repeat standardized assessments and monitoring activities.  We have worked with several collaborative partners on QI projects to either help them implement such capabilities locally or outsource it to us.
