HQP's Model is Vital to Medicare

Improving our nation's health insurance is important, but it doesn't begin to address the problems with our care delivery system that are driving up costs and resulting in poor outcomes.  With Medicare hurtling toward insolvency and preventable complications of chronic illness rampant, HQP's model of community-based advanced care management represents a breakthrough that can help address these root causes - right now.

Evaluators for the Medicare Coordinated Care Demonstration (MCCD) report that among a group of high-risk beneficiaries followed over several years, the HQP program reduced hospitalizations 24% (p=0.005) and lowered hospital costs $255 per person PER MONTH (more than $3,000 per person per year). [1]  The policy implications for Medicare are significant since 18.1% of ALL Medicare beneficiaries nationwide in 2003 were members of this risk group.

In another report by the same analysts, HQP's model was found to have significant cost savings across a wide variety of higher-risk groups and was the only MCCD program that significantly reduced deaths (p=0.036). [2]  More results will be published soon about the positive impact HQP's program has on health outcomes, but the opportunity for Medicare to address significant root causes of high cost and poor quality among chronically ill older adults using HQP's program is already clear.

HHS / CMS Announce Multi-payer Advanced Primary Care Practice Demo (MAPCP demo) for States

Jun 2, 2010 - An ambitious demonstration was announced challenging States to transform primary care into a more potent system of care to improve outcomes and reduce costs.  The most demanding requirements are ones that HQP already has extensive experience in delivering.  Read more

  

HQP Presents at Symposium on Reducing Readmissions

May 26, 2010 - Sherry Marcantonio and Ken Coburn of HQP presented at a symposium in Philadelphia launching a collaborative to address key aspects of discharge and care transitions processes.   The conference was attended by over 200 people, representing 40 hospitals and health systems and 20 non-acute providers and community organizations. Keynote speakers included HQP Board member Mary Naylor, PhD, RN, of the University of Pennsylvania.  Learn more

  

Coburn recieves award for health leadership from UNC

Apr 23, 2010 - Ken Coburn, MD, MPH, CEO and Medical Director of HQP, was awarded the Arthur B. Holzworth endowed scholarship in health leadership from the UNC Gillings School of Global Public Health.  The award is given to an outstanding student in the executive Doctoral Program in Health Leadership (DrPH).
  

Aetna and HQP collaborate to improve care to Medicare Advantage members  

Dec 7, 2009 - Aetna and HQP have completed an agreement making HQP's community-based advanced care management program available to certain Medicare Advantage members in eastern PA.

CMS Provides a Short-term Extension to HQP while Considering Next Steps

Feb 22, 2010 - Recently, CMS issued HQP a short-term extension to continue providing services to Medicare beneficiaries enrolled in the Medicare Coordinated Care Demonstration.  The purpose of this extension is to sustain HQP's work with fee-for-service Medicare beneficiaries while CMS considers how continued study of HQP's model could lead to helping chronically ill Medicare beneficiaries on a larger scale. Congresswoman Allyson Schwartz and Congressman Patrick Murphy were instrumental in collaborating with CMS on ways to advance this work.  The offices of Senators Casey and Specter also supported the review process.
