HQP is grateful for the privilege to work with these exceptional leaders in health care. - Doylestown Hospital - HQP has worked with Doylestown Hospital for several years to assist the hospital to develop an Improving Systems model for organizing and enhancing quality improvement and work redesign in various areas throughout the hospital. In addition, this visionary community hospital has provided HQP significant and committed support - superb HR and employment support, senior team advice and networking, and collaborative visioning of the future direction health
care reform should take. HQP would not be here today without the support of this great community resource.
- Center for Medicare and Medicaid Services (CMS) - Staff within the Office of Research, Development, and Information have been invaluable partners in our successful implementation of the Medicare Coordinated Care Demonstration (MCCD).
- Aetna - is one of the nation's leaders in health care, dental, pharmacy, group life, and disability insurance, and employee benefits. In December 2009, HQP and Aetna began a collaboration to evaluate HQP's community-based nurse care management model as an additional service to certain chronically ill Medicare Advantage members in eastern Pennsylvania.
- The NewCourtland Center for Transitions and Health, and Penn Nursing Research - HQP has embarked on an exciting collaborative research partnership with Mary Naylor, PhD, RN, Director of the NewCourtland Center for Transitions and Health and a pioneering researcher in several areas of the science of nursing and quality. Dr. Naylor is perhaps best known for her work on the Transitional Care Model. She is also a Board member of HQP. It is an extraordinary opportunity for HQP to work with a world-class research team.
- SelectHealth (NYC) - SelectHealth is an innovative Special Needs Plan (Medicaid) offered by the New York-Presbyterian Health System caring for those with HIV/AIDS. HQP worked to assist SelectHealth in utilizing their considerable data to improve efforts to support their members to maintain adherence to medication regimens, preventive services, healthy behaviors, and follow-up with their heatlhcare providers.
