Aetna and HQP to collaborate
After more than 2 years of hard work on both sides, HQP and Aetna have reached an agreement to provide HQP’s community-based nurse care management to certain Medicare Advantage members with chronic illness. The service will be
available to eligible members of participating practices in a region of eastern Pennsylvania.
We are excited to have the opportunity to replicate our success in the Medicare Coordinated Care Demonstration with Aetna members and look forward to working with our colleagues at Aetna.

Does your agreement with Aetna mean I can continue to receive care management through this collaboration? Due to financial considerations,
I had to cancel my supplementary insurance on 12/31/09 and sign up with Aetna’s HMO Basic Plan.
For the past several years, I have benefited by having Stephanie Weisner of your Doylestown, PA unit monitor my health. She provided excellent care! I have a condition called myocardial ischema and have recently had 2 drug -eluting stents implanted.

Ed,
Thanks for your kind words about our program. Yes, with our agreement with Aetna, it may be possible for you to again receive nurse care management support from our team. It will depend on who your primary care provider is and whether their office is participating in the Aetna program and your current clinical needs. Please call our office 267 880-1733 ext 22 to speak with Stephanie and she will help figure this out.
Thanks again,
